I must have missed this somehow when it was first posted, but how wonderful to stumble across it later! Girlchester fic doesn't always quite work for me, but you also have a grasp of the Winchesters like no other, and I think you nailed all the little things that would have been different for her had Sam been a girl. But also she is SO SAM with her belligerent conviction that that things would be different if she were a guy (like Dean). "She wouldn't feel this life like a bear trap, its steel teeth slowly closing around the soft skin of her ankle while she just stands there dumber than the dumbest wild beast and lets it." is an amazing line. So much of this - Sam's intelligent but awkward adolescence, Dean's presence, her very teenage (but intensified by the nature of their lives) struggle with identity - is amazing, really.
And the way Sam looks at Dean, Dean-her-brother, is exquisite. Their relationship and that it is so familiar and comfortable and fraternal, and then with the slow creep of Sam's UST beneath, is the best part of this fic for me.
