This is pretty much the only episode where we've seen Sam--and I'm talking real Sam, not Sam-possessed-by-a-demon or Sam-overrun-with-aggression-because-of-the-experiments-of-an-evil-ghost--this was the only time I can ever recall seeing Sam use the full force and power of his body to be aggressive. Menacing. Threatening. He full-on lost his shit when Dean was dead/caught in a dying loop.
I think maybe this episode could hold a little bit of foreshadowing, in that respect. Maybe it's meant to show us that Sam can and will go darkside without Dean. Dean is the only one anchoring Sam; without Dean's presence, his innate goodness, Sam isn't Sam anymore. And, yeah, that ruthless badassery was fucking HOT, but it was also scary and strange and wrong and so terribly, terribly saddening.
I think this was meant to show that Sam will do anything--and I mean anything--to keep Dean safe.
